A Life of Curiosity and Compassion with Dr. Frank Alvine

Dr. Frank Alvine, a 2009 South Dakota Hall of Fame Inductee, built a career defined not just by innovation but by a deep desire to solve problems and improve the lives of others.

From his early days in Sioux Falls to operating rooms across the world, his journey is one of steady dedication. He was never interested in accepting things as they were. If something could be better, he believed it was worth the effort to find a way.

At its core, his story is not just about medicine. It is about persistence, curiosity, and the people who shape us along the way.

The Support That Shaped a Career

Frank’s story begins in Sioux Falls, in a home that was often full and with little means. Space was limited, but expectations were not. Hard work was simply part of daily life.

As a young boy, he spent his summers working on his uncle’s farm. The days were long, the work was tough, and there was little room for complaint. But those experiences stayed with him. They taught him something simple and lasting. When you start something, you finish it. This mindset carried him forward, even when life became uncertain.

While pursuing a medical career, financial hardship forced Frank to step away from the two-year medical school program at the University of South Dakota(USD). For a time, the future he had been working toward felt out of reach. Then, in a moment he still remembers clearly, a mentor stepped in and helped him find his way back. 

Dr. Hard, the Dean of the University of South Dakota Medical School at the time, was sincerely committed to his students. When he saw the promise of a bright future in medicine for Dr. Alvine, he called him up and asked him to return to school, waiving his tuition costs. 

This support was a turning point for Dr. Alvine. A reminder that sometimes, having someone believe in you can change the course of your life. 

A Medical Breakthrough

Dr. Alvine completed medical school and built his career as an orthopedic surgeon. A tenet of his practice was to ask questions others had stopped asking. At the time, patients with severe ankle conditions had few reliable options. Early replacement systems often failed, and many in the field had moved on from the idea altogether.

But he had not. Instead, he leaned in, studying what existed, testing ideas, and working through failure after failure. Progress did not come quickly, but it did eventually come, and it changed lives for the better.

Dr. Alvine’s steady dedication led to the development of the Agility Total Ankle Replacement System, which received FDA approval in 1993 and changed what was possible for patients.

The number of patients Dr. Alvine helped during the course of his career is impressive. Dr. Alvine has performed more than 1,500 procedures and trained surgeons around the world.

But those are not the moments that have stayed with him.

It was the patient who could return to work. The farmer who could get back into the field. The simple act of walking without pain.

“Knowing we could really make a difference in people’s lives,” Dr. Alvine shared, “that is something I am very proud of.”

A Guiding Belief

Throughout his life, Dr. Alvine has been guided by a quiet but steady philosophy.

Learn deeply, stay focused, and just keep going.

He never viewed challenges as stopping points. Instead, they were opportunities to understand more.

“The more you know about a subject, the easier it becomes,” he said. “What you thought was insurmountable is really not.”

When asked how he hopes to be remembered, his answer is simple. Not for titles. Not for recognition. But for honesty, integrity, and trustworthiness.

A Lasting Influence 

Even in retirement, Dr. Alvine’s influence continues to reach far beyond the operating room.

His work still shapes how surgeons approach complex procedures today, but perhaps even more meaningful is the way that impact has carried forward within his own family. His son followed in his footsteps, becoming a surgeon and eventually joining the same practice. For a time, they worked side by side, sharing not only a profession but a passion for helping others. It is a continuation of his life’s work in its most personal form.

At home in South Dakota, his focus has also turned to conservation. He has dedicated time and energy to preserving wildlife habitats and restoring natural spaces, driven by a desire to protect what makes the state special.

From medicine to the outdoors, his approach has remained unchanged. Do the work. Stay curious. Care deeply.
